Amber Valley CVS Social Prescribing Service is a free service being provided in partnership with the Alfreton, Ripley, Crich and Heanor (ARCH) Primary Care Network. The aim of the service is to work alongside G.P. practice teams and community organisations to help you explore activities and local support that could improve your health and wellbeing.
Social prescribers are available to connect you to local services to improve your physical and mental wellbeing, including:

Healthy Lifestyle | Support Groups |
You can self-refer to this service without seeing or speaking to a GP first.
The social prescriber works closely with the care co-ordinators and health and wellbeing coach and other members of the primary health care team.
