Improvements to Diabetes Reviews

The practice is making positive changes to how it monitors and supports patients living with diabetes.

As part of each patient’s annual review, they may be invited for an initial assessment where key health information—such as BMI, blood pressure is recorded and and a blood sample is taken. This ensures that every patient receives a thorough health check before their appointment with the nurse, during which they will have the opportunity to discuss areas such as medication and overall diabetes management.

To support patients whose diabetes is not currently well controlled, the practice may also invite them for interim reviews in addition to the annual review. This means some patients may hear from the practice more frequently than before.

Why is this happening? 
The aim is to provide patients with the best possible support in managing their diabetes. More frequent checks help the practice:

  • Spot any changes earlier
  • Review treatment plans more regularly
  • Provide timely advice and personalised care
  • Work together with patients to improve long‑term health

What this means for patients

  • They may receive additional messages, reminders, or appointment‑booking links
  • They may be asked to have blood tests a little more often

The practice recognises that life is busy and aims to make these appointments as convenient and useful as possible. Each review helps the team understand how patients are getting on and what extra support they may need.
