The practice is making positive changes to how it monitors and supports patients living with diabetes.
As part of each patient’s annual review, they may be invited for an initial assessment where key health information—such as BMI, blood pressure is recorded and and a blood sample is taken. This ensures that every patient receives a thorough health check before their appointment with the nurse, during which they will have the opportunity to discuss areas such as medication and overall diabetes management.
To support patients whose diabetes is not currently well controlled, the practice may also invite them for interim reviews in addition to the annual review. This means some patients may hear from the practice more frequently than before.
